Client Agreement

Individual Therapy Sessions

This Client Agreement covers the terms and conditions for individual therapy services provided by Sublime Focus, facilitated by Jeremiah Hundley. By proceeding with these services, you confirm that you have read, understood, and agree to the terms outlined below.

Scope of Service

Under this agreement, you will receive customized therapeutic services to address your unique goals. The package includes:

  1. Free 20-Minute Consultation
    • Conducted online or by phone
    • Purely exploratory; no obligation to book further sessions
    • Valid for 30 days from initial contact
  2. 90-Minute RTT Session
    • Tailored Rapid Transformation Therapy session targeting your specific objectives
  3. Two Follow-Up Calls
    • 7-Day Follow-Up: Discuss initial insights, provide further guidance, and address questions
    • 21-Day Follow-Up: Review ongoing progress and offer additional support
  4. Transformation Recording
    • Custom recording to reinforce positive changes initiated during your RTT session

Client Responsibilities

  1. Scheduling Follow-Up Calls
    • Arrange your 7-day and 21-day calls within those respective timeframes
    • Missed calls may not be rescheduled unless otherwise agreed
  2. Daily Listening
    • Listen to your transformation recording every day for optimal results
    • Recommended times include:
      • Before sleep
      • Upon waking
      • During a quiet meditation session

Booking and Payments

  • Payment Methods: PayPal, major credit cards, Bitcoin, or in-person cash (by prior arrangement)
  • Full Payment: Must be made before the session to confirm your appointment
  • Receipts: Provided upon request for all payment methods

Informed Consent

By participating in therapy with Sublime Focus, you acknowledge that:

  • Not a Replacement for Medical Treatment: RTT does not replace medical, psychological, or psychiatric care
  • Results May Vary: No specific outcomes are guaranteed; personal effort and engagement influence results
  • Supplementary Guidance: Any advice or resources offered complement, not replace, professional medical care

Contact Between Sessions

  • Office Hours: 9:00 AM – 5:00 PM, Monday–Friday
  • Communication Channels: Phone, email, or mail
  • Response Time: Messages received after hours are addressed on the next business day

Confidentiality

All therapeutic interactions—phone calls, emails, recordings—are handled in strict confidence. Session recordings require your prior agreement and are retained securely. Confidential information may be disclosed only under the following circumstances:

  • Explicit Client Consent: You authorize sharing with a specific individual or agency
  • Legal Requirements: Court orders, subpoenas, or mandatory reporting
  • Risk of Harm or Vulnerability: Potential harm to yourself or others, or suspected abuse/fraud
  • Professional Reporting: A referring healthcare professional requests a report (copy available upon request)
  • Supervision and Training: Anonymized case details may be discussed with colleagues; identifying information remains confidential without your permission

Session Recordings

  • Personal Use Only: Any recordings provided are exclusively for your own personal use
  • Prohibited Sharing: Recordings must not be shared, copied, sold, or distributed
  • Safe Listening: Do not listen while driving or operating machinery

Privacy Policy

Your personal data is collected, processed, and stored according to the Sublime Focus Privacy Policy. This includes booking details, session records, and any relevant communication. Data is not shared without your explicit consent, except as noted in Confidentiality.

Medical or Psychological Conditions

To ensure the best care possible, you agree to:

  • Disclose Relevant History: Share any medical or psychological conditions that might impact therapy
  • Update Changes: Inform Sublime Focus of significant health updates
  • Obtain Clearance (If Needed): Provide written consent from a healthcare professional upon request

Note: Services are unavailable to clients with epilepsy or psychosis without explicit medical clearance.

Age Restrictions

  • Adult Clients (18+): Eligible for online sessions
  • Minor Clients (Under 18): Must be accompanied by a parent or guardian for in-person sessions

Attending Your Sessions

  • Timeliness: Arrive on time or be ready at your scheduled online slot
  • Cancellations: See Cancellation, Rescheduling, and Refund Policy for details

Cancellation, Rescheduling, and Refund Policy

  1. Notice Periods
    • In-Person Sessions: 48 hours’ notice required
    • Online Sessions: 24 hours’ notice required
  2. Refunds
    • No refunds for late cancellations or attended sessions
    • Prepaid packages may be pro-rata refunded, minus the session fee for any used sessions

Outcome of Sessions

  • No Guarantees: Every individual’s results differ
  • Active Participation: Success depends on your engagement and the techniques provided

Standards of Behavior

  • Respectful Conduct: Both client and facilitator share mutual respect and courtesy
  • Substance-Free Attendance: Sessions must not be attended under the influence of alcohol or recreational drugs
  • Termination: Sessions will be canceled if you attend under the influence or behave abusively; no refunds provided

Hypnotherapy Recordings

  • Personal Use Only: Recordings are strictly for your own home practice
  • Safety Precautions: Do not listen while operating machinery or vehicles
  • Daily Practice Recommended: Maximize results by following the suggested listening schedule

Liability

  1. Limitations

    Sublime Focus and Jeremiah Hundley are not liable for direct, indirect, incidental, special, punitive, or consequential damages, including lost profits or revenues.

  2. Applicability

    This applies to any damages arising from advice, resources, or services provided during therapy, whether in contract, tort, or otherwise.

  3. Client Responsibility

    You agree to defend, indemnify, and hold Sublime Focus harmless from any claims or losses resulting from your participation. You assume full responsibility for the application of any techniques learned.

Governing Law

This agreement is governed by the laws of Los Angeles, California. Any disputes fall under California courts.

Concerns and Complaints

If you have concerns regarding your therapy experience, address them directly with Jeremiah Hundley. We strive to resolve issues promptly and professionally.

  • Contact Method: Email or phone during office hours
  • Response Time: Acknowledgment within two business days
  • Escalation: If unresolved, you will be informed of next steps and potential third-party mediation

Updates to This Agreement

Sublime Focus may update these terms without notice. Please check periodically for revisions.

Statements of Understanding

By booking an appointment, you confirm the following:

  • Scope of Therapy: You have been informed of the services offered by Sublime Focus and agree to participate.
  • Outcome Variability: Results may vary, and no specific outcomes are guaranteed.
  • Medical Responsibility: Therapy is not a replacement for licensed medical or mental health treatment. You will consult a healthcare professional for any medical concerns.
  • Clearance: You agree to obtain written approval from a licensed professional if requested.
  • Concerns and Communication: You will communicate any issues in a professional manner, following the procedure in Concerns and Complaints.
  • Autonomy in Therapy: You may end sessions at any point at your discretion.
  • Personal Responsibility: You assume responsibility for applying any advice or techniques offered.
